Comprehensive eyewear Cover

How it works

Iris Comprehensive Cover provides your members with eyewear cover against loss, theft and accidental damage. Cover is based on how many claims your member is permitted to make in any twelve month period. The number of available claims will depend on your specific membership level benefits.

Terms and Conditions

  • Number of available claims in any twelve month period depends on your specific membership level benefits

  • Eyewear being claimed for must be under twelve months old

  • Provides cover against loss, theft and accidental damage

  • A valid crime reference number may be required for theft claims

  • Covers eyewear up to a value of £1000

  • 10% excess (based on original purchased price) payable directly to Iris Visioncare by the member

  • Settlement of your costs is subject to successful submission of all relevant supplier invoices, proof of purchase and a completed claim form

  • A £75 dispensing fee is included in all settlement payments

  • Iris Visioncare reserve the right to decline any claim applications that fail to meet these terms and conditions 

Processing claims

Iris Comprehensive Cover provides your members with eyewear cover against loss, theft and accidental damage. Cover is based on how many claims your member is permitted to make in any twelve month period. The number of available claims will depend on your specific membership level benefits.

Print two 

Claim Forms

Obtain proof of purchase

for the eyewear

Discuss how the loss, theft

or damaged occurred

Original order form/invoice or

itemised till receipt x 2

Complete both claim forms

Attach proof of purchase to each claim form

Create an order for repair

or replacement eyewear

Do not send the new order off until you have received the go ahead from Iris

Send one of the completed claim forms and proof of purchase to Iris

Scan and email them to Iris or

post them to Iris

Retain the duplicate copies

for later on

Put these with your patient records

Once we receive the claim form, we will carry out further checks and contact the member to arrange payment

of the 10% excess

If the claim is approved, we'll get in contact to give you the go ahead to proceed with the new order

Member collects replacement or repaired eyewear

Retain all supplier invoices

that relate to the new order

Complete the "Claim back my Costs" section on the

duplicate claim form

Send the duplicate claim form and invoices to Iris

Iris will normally make a settlement payment within 7 days via BACS

Settlement payment will include a £75 dispensing fee
